Medical Alert

Our agency responds to several medical calls each month where either: 1) the patient and their family couldn't remember their medical history and medications; or 2) precious time was used while manually writing down the patient's medical history and medications.  Unfortunately we encounter this situation more and more often.

The absolute best method of assisting us → paramedic units → the emergency room is to have your information typed on a wallet-card or a full-sized sheet of paper (although hand-written information is far better than NO information).

Please consider the following information.  Note that the graphic provides a website to download forms from.  To make things easier, we are hosting them on our site.  They are available to download at the bottom of the page.  We strongly encourage you to use them for each member of your household.

  2. Open the file(s) in Adobe Acrobat Reader or similar PDF reader software.
  3. Enter the medical information requested and click SAVE.  Saving makes it easier to edit your information and print new copies as needed.

  4. Print your card/form.  Keep one in your wallet, car and on your refrigerator.  Each person in the household should have their own cards and forms.
